Darling Downs and West Moreton PHN (PHN) is a not-for-profit organisation that works at a regional level to build healthier communities through a connected, sustainable and responsive primary health care system.

Our role is to support primary care and service providers to ensure the right care is there for those who need it most. Where needed, we commission additional services to enable equitable health and wellbeing.

About the role:

We are seeking an experienced and values-driven Director to contribute to the development and implementation of the PHN's strategic framework, performance accountability mechanisms and organisational capability development to ensure the organisation delivers on its strategic objectives.

The role will lead the effective planning, design, implementation and monitoring and evaluation of primary care transformation and system integration initiatives, ensuring activities are aligned with the organisation's strategic plan, are evidence based, meet population health needs and address service gaps and access issues.

The role will also lead and build the capability of the primary care team to enable general practices and allied health to deliver high quality and accessible care through support, resourcing and driving performance.

Key Responsibilities

Provide strategic and operational leadership to enable primary care transformation including needs assessment, priority setting, and innovative solutions to workforce, service integration and coordination,



quality improvement and emergency preparedness.

Lead the ongoing development and implementation of the Healthy Primary Care plan and deliver on primary care transformation and health system integration initiatives.

Lead the operations of the primary care team, providing direction, development and coaching to achieve a high functioning team.

Work collaboratively with a wide range of stakeholders to identify service gaps and access issues and implement evidence-based solutions to address these in collaboration with health system partners, fostering integration at the system level.

Build and maintain relationships with key stakeholders including GPs, Allied Health, Hospital and Health Services, government bodies, national organisations, and local services.

Provide advice and thought leadership to inform the development and implementation of innovative evidence-based initiatives in collaboration with stakeholders.

Manage and monitor program reporting, human resources, budget, and service risks/issues across the Primary Care Team with a diverse portfolio including GP engagement initiatives, Urgent Care Centres, Team based care projects, Quality Improvement activities and Disaster Management.

Function as an effective member of the Leadership Team,



and work collaboratively across the organisation to support its strategic objectives.

About You

You are an accomplished senior leader with experience in health, human services, community services or public health planning.

You will bring:

Extensive experience in leading complex and long-term change in the health or community setting.

Exceptional people leadership and operational management skills.

Relevant tertiary qualifications and experience in health, social services, and/or health management and planning.

High level knowledge and understanding of contemporary primary care models and reform opportunities.

Demonstrated systems thinking approach supported by strong analytical and problem-solving skills.

Ability to nurture solid working relationships and drive change processes with internal and external stakeholders including government, non–government and primary care sectors.

Highly developed written and verbal communication and interpersonal skills.

Ability to apply program and project management tools, processes, and methodologies.
