Summary of Position

The Diabetes Educator position is well suited to someone that is a Credentialled Diabetes Educator with the Australian Diabetes Educators Association (or working towards), with an understanding of Aboriginal cultural values and commitment to the advancement and self-determination of Aboriginal communities.

The Diabetes Educator will integrate diabetes self-management education with clinical care as part of a therapeutic intervention to promote physical, social and psychological wellbeing. The role will also encourage the client to consider a comprehensive self-management plan that will maximise their health outcomes. A key responsibility will be to support and encourage health centre clinical staff through monitoring of progress toward desired goals, ensuring appropriate referral pathways are in place while acting as a resource person for staff.

Your responsibilities will include (but are not limited to):

- Provide evidence-based and culturally appropriate education to clients and their carers across the KWHB region to support the recognition of health risks, diabetes management and the self-management of chronic disease,



while building trusting and respectful relationships with individuals, families and communities.
- Work collaboratively alongside Aboriginal Health Coaches to support coordinated, culturally responsive chronic disease care, including client education, self-management support, health promotion, follow-up and ongoing engagement with clients and families.
- Provide education and counselling using an empowerment approach and education to clients, carers, and families of those living with diabetes.
- Liaise with other members of the Primary Health Care team together with the client to identify any barriers to supporting clients in the management of diabetes and develop strategies to address these.
- Support and participate in quality improvement, data collection and reporting activities to support the provision of protected, effective, and high-quality health care.

In return, you'll receive an attractive remuneration package (negotiable based on your skills and experience) from $127,428 - $138,285 (RN1- RN4 classification) or $73,108-$169,990 (HP1-HP4), plus 12% superannuation and a range of benefits, including:
