About the role

The Coordinator of Individualised Services will manage a range of support services to assist people with disabilities to live the "good life".

Key responsibilities

- Act in a person centred manner at all times
- Ensure through coordination and leadership that GIFSA is delivering high quality supports and services
- Assume responsibility for the people we support's roster and staffing matches
- Regularly review the services and supports provided to the people we support, ensuring their needs are being met
- Utilise effective work practices to encourage choice and control for the people we support
- Ensure individual plans are person centred and reviewed as per deadlines
- Build and maintain relationships through quarterly contact with the people we support and all relevant stakeholders
- Ensure individual and group risk assessments are complete and documented where required
- Build and maintain effective teamwork between support staff
- Provide direction and mentoring to support staff, ensuring they are provided with feedback, coaching, training and support to improve performance

About you

- Relevant experience in the Human Services industry
- Ability to manage and lead a team and team leaders
- Well-developed advocacy skills
- Ability to build and nurture relationships at a personal,



organisational and community level
- Effectively manage and implement medically developed care plan requirements as directed by qualified professionals and specialists
- Ability to set goals, manage a variety of tasks and achieve outcomes
- Demonstrated computer skills including a working knowledge of databases, Microsoft Excel and Microsoft Word
- Well-developed problem solving skills
- Well-developed communication and interpersonal skills and the demonstrated capacity to work effectively within a team
- Current First Aid Certificate, Current Working with Children Check, NDIS Workers Screening Check, and Current WA "C" class drivers licence
