HammondCare is seeking 3 x 0.6FTE Registered Nurses to cover the Central West/Nepean/Blue Mountains region. The Clinical Partner – Registered Nurse will join our Home Care Clinical Care & Practice Team, delivering person-centred care in clients’ homes and in Seniors Living units.
You will assess needs, set goals, and provide treatment and interventions, including home visits or group activities, with support from an experienced multidisciplinary team.
