Fair Foundations is seeking a Support Coordinator to provide person-centred, trauma-informed capacity-building support to NDIS participants, including those with complex needs. You will help participants understand and implement their plans, address barriers, connect with mainstream, community and provider supports, and build confidence to coordinate and manage services.

Reporting to the Head of Disability Services, the role includes preparing participants for plan reviews, coordinating funding
