About this opportunity:

Reporting to the Head of Disability Services, the Support Coordinator provides person-centred, trauma-informed capacity-building support to NDIS participants, including people with complex needs. You will help participants understand and implement their plans, address barriers, connect with mainstream, community and provider supports, and build confidence to coordinate and manage their services.

Key responsibilities include supporting participants to prepare for plan reviews, arranging assessments, coordinating service agreements and funding allocations, maintaining accurate case notes and reporting, building stakeholder relationships, and developing a sustainable Support Coordination caseload that meets agreed targets.

About you:

You bring a relevant Certificate IV, Diploma or Advanced Diploma in Disability, Community Services, Mental Health, Welfare, Mental Health Peer Work or Counselling,



together with at least three years’ experience providing support coordination to people with complex needs. Understanding psychosocial support needs and ability to provide practical and recovery- oriented support for people is highly regarded.

You have sound knowledge of the NDIS Quality Standards and principles, experience implementing participant plans, and the ability to build effective relationships with participants, families, providers and community stakeholders.

You will also have high-level communication skills, experience working with culturally and linguistically diverse communities, current First Aid and CPR certificates, a valid Class C NSW Driver Licence with access to a vehicle, and a current NDIS Worker Screening Check. A current Working with Children Check, or willingness to obtain one promptly, is also required.
