Pearl Home Care - Sydney Inner West is seeking a Community Worker to provide person-centred support to older Australians living at home. You will assess client needs, coordinate services and implement support plans to promote independence and safety.

You will liaise with healthcare professionals, maintain case notes and prepare reports to support care planning and ensure services meet quality standards. This is a community-based role with emphasis on compassionate, patient-focused care.
