As a Care Partner based in our Aspley office, you will play a vital role in supporting customers across a range of funding programs, including Commonwealth Home Support Program (CHSP), Support at Home, Veterans' Home Care, Nursing Services, NDIS, and private fee-paying services. Working closely with clients, families, caregivers, and service providers, you will coordinate and monitor services that meet individual needs while ensuring compliance with the Aged Care Quality Standards.

This is a full-time position, working Monday to Friday between 8:00am and 4:00pm.

About You

- Experience in aged care, community care, disability services, health administration, or care coordination
- Excellent verbal, written, and interpersonal communication skills
- Ability to work independently while contributing positively within a team environment
- Strong organisational skills with a high level of attention to detail
- Ability to manage competing priorities and maintain accurate client records
- Commitment to supporting clients to live fulfilling, independent lives
- Knowledge of Aged Care Quality Standards and best-practice service delivery

Key Responsibilities

- Coordinate the delivery of high-quality care and support services to clients




- Conduct onboarding for current clients across various funding streams
- Complete regular care plan reviews and respond to clients' changing needs
- Build positive relationships with clients, families, caregivers, and stakeholders
- Respond to enquiries, referrals, and follow-up actions via phone and email
- Monitor client budgets where applicable
- Partner with external providers to ensure services are delivered smoothly and effectively
- Maintain accurate and timely client records and documentation
- Provide caregivers and nominated support persons with feedback, updates, and ongoing support
- Ensure services align with client goals, organisational requirements, and Aged Care Quality Standards

Requirements

- Qualification in Health, Community Services, Disability, Aged Care, Business Administration, or a related discipline (for example, Certificate III Individual Support or Nursing qualification)
- Strong knowledge of Aged Care Quality Standards
- Experience, or willingness to learn, in the management of government-funded care packages, community resources, and relevant aged care legislation
- NDIS Worker Screening (or willingness to obtain)
- Current Police Check (issued within the last 6 months)
- Current First Aid & CPR Certificate
- Current Queensland Driver Licence and access to a reliable vehicle
- Full working rights in Australia
