Join Senior Helpers Northern Beaches and Suburbs, a well-established and growing Support at Home provider, and be part of a dedicated team focused on delivering high-quality, client-centred support to older Australians.

This is a Part-time role based at our Dee Why Office.

Your day to day will include...

- Conduct holistic client assessments and develop care plans that support their needs, maximise client choice, well-being and independence.
- Conduct regular monitoring of services and care plan reviews to ensure optimal care and services are maintained to meet changing needs.
- Co-ordinate services and organise supports based on government compliant care plan for the client.
- Communicate effectively and build trust with clients and their family or representatives.
- Responsible for management of your own portfolio of clients for their care, Support at Home budgets and their relevant documentation. Portfolio will consist of reasonable number of clients only.
- Liaise and work closely with internal team members in office-Enrolled Nurse, Registered Nurse, scheduling and admin.

About you...

- Proven background in care co ordination /managing clients within Support at Home / Home Care Package or NDIS programs
- Strong communication abilities that help you build trust with client's family and work confidently with people from wide-ranging backgrounds
- Highly organised, with excellent time management and strong attention to detail
- Understanding of the Support at Home framework or training can be provided
- Comfortable using technology,



including digital care management platforms
- New graduates EN's are encouraged to apply if past experience in aged care.

Licence/Certification:

- Diploma in Nursing or Diploma in Community Services
- Or Bachelor of Social Work
- Or Certificate IV in Ageing and Disability Support
- National Police Certificate
- First Aid and CPR certificates
- Unrestricted NSW Driver License

Work Authorisation:

- Australia (Mandatory)

Pay: $40.60 – $45.00 per hour
