Care Coordinator – Full Time Ongoing
Community Services #1 is seeking two experienced and passionate Care Coordinators to join our team. As a Care Coordinator, you will facilitate quality service delivery across our Community Assistance and Temporary Supports (CATS) Program, Out of Hospital Care, and Aged Care services.
About the role:
- Conduct client intake, assessments, and support planning to identify individual needs, goals, and appropriate service pathways.
- Coordinate and facilitate access to Community Assistance and Temporary Supports (CATS), Out of Hospital Care, Aged Care, and other relevant community services.
- Monitor client progress, review support plans, and provide ongoing care coordination to ensure positive outcomes and continuity of care.
- Build and maintain effective relationships with clients, families, carers, health professionals, and community partners while ensuring accurate documentation and compliance with service requirements.

About you:
- Certificate IV/Diploma qualification in Community Services, Disability Support, Aged Care, Social Work, Health Services or a related field, with demonstrated relevant experience
- Demonstrated experience in care coordination, including assessment, planning, monitoring, adjusting support plans, and managing participant transitions or exits.
- Experience delivering high‑quality telephone and face-to-face assessments, referrals,
and service navigation for individuals with diverse needs.
What we offer This is a full time, ongoing role working 76 hours per fortnight. Remuneration is aligned with the SCHADS Award Level 4, plus superannuation and salary packaging benefits.

Requirements
Current driver’s licence and ability to obtain/maintain required pre-employment checks and certifications (including National Police Check, Working with Vulnerable Card and First Aid), plus mandatory training as required.
