Greatness starts here We have an exciting opportunity available within the Health and Wellbeing Program our Seniors Lifestyle Victoria Program team that will provide seniors with care coordination at DPV Health by delivering care coordination assessments,



education and support to clients with mild to moderate frailty and Improve integration and coordination between Primary, Community and Acute Care Services.

Seniors Lifestyle Program care coordinators will act as the key point of contact for Seniors Program clients. Seniors Program coordinators will:

- Support with planning and implementation of the CH Seniors Lifestyle Program
- Undertake a needs identification and risk assessment for enrolled clients.
- Work with enrolled clients to develop individual care plans.
- Connect clients with local services and supports.
- Liaise with clients’ GP and other care providers.
- Support clients to implement individual care plans, which may include:
- support managing appointments, providing prompts and reminders.
- assisting with appropriate referrals for medication management
- facilitating material supports
- arranging transport
- addressing concerns with care arrangements

- Monitor clients for deterioration
- Respond or facilitate response to monitoring and escalate care as required.
- Maintain full and complete client records and data sets for reporting purposes as part of the funding and research requirements for the CH Seniors Program pilot.

The care coordinators must be a suitably skilled allied health professional.

Desired Skills and Experience

Bring your great, everyday! You will have current registration with the Australian Health Practitioners Registration Agency (AHPRA) or other relevant registration body.

You will also be required to have the following in order the exceed in the role:

- Experience working with clients with complex health needs, social issues, and complications of chronic disease.
- Knowledge and ability to assess and manage clients experiencing frailty




- Well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with client,
- Experience working as an allied health clinician in a community setting including work with CALD, Aged and clients with a disability.
- Experience working as an allied health clinician in a community setting engaging with external agencies and services.
- Experience in planning, implementing, and evaluating cutting-edge, evidence based, clinical services.

The fine print

- Successful candidates will be required to undergo a National Police Check, hold a valid WWCC and NDIS Worker Screening Check and provide evidence of full Covid-19 Vaccinations or relevant medical exemption.
- Full driver’s license and travel between sites is required.
