About the Opportunity

Our client is a respected home care provider and are seeking a Clinical Care Partner to join their growing team. This is a community-based role where you will manage, coordinate and deliver evidence-based clinical care to older Australians living at home, helping them achieve their goals under the Support at Home Program. This is a community-based role involving home visits and travel in Perth.

This role will suit a clinician who enjoys combining hands-on care with case management, and enjoys seeing the real difference your work makes in people's lives.

Key Responsibilities:

Reporting to the Clinical Manager, you will:

- Conduct initial and ongoing holistic assessments, and develop, implement and review individualised care plans
- Deliver clinical interventions, such as wound care, medication administration, chronic disease monitoring, continence management or reablement planning
- Monitor client health, identify risks and escalate deterioration promptly
- Support clients through a safe transition home after hospital discharge
- Coordinate input from GPs, allied health and specialists
- Monitor care utilisation to make the most of each client's Support at Home budget
- Oversee rostering and scheduling of support services with the wider team
- Provide clinical mentoring and delegation to Enrolled Nurses, Therapy Assistants and Support Workers
- Contribute to audits, incident reviews,



complaints and continuous improvement in line with the Strengthened Aged Care Quality Standards

About You:

- Bachelor's degree in Nursing

- Current AHPRA registration as a Registered Nurse

- Demonstrated experience in aged care or community nursing

- Case management or clinical coordination experience in aged care or disability
- Knowledge of the Support at Home Program, aged care reforms and relevant legislation
- Experience delegating and monitoring care delivered by others

- Leadership skills, including delegation, feedback and addressing performance
- Strong clinical judgement, problem solving and decision making
- Excellent interpersonal skills and the ability to build relationships with clients and referral sources
- Ability to manage multiple priorities in a fast-paced environment
- Current First Aid and CPR and National Police Clearance

- NDIS Worker Screening, and a driver's licence
