About This Role

Your new role

As a Clinical Nurse within the Multidisciplinary Memory Clinic, you'll provide advanced clinical expertise and leadership in the assessment, care planning and management of older people with complex cognitive and geriatric care needs. Working within a multidisciplinary outpatient setting, you'll coordinate specialist nursing care, support evidence-based practice and contribute to education, quality improvement and research.

- Provide expert, person-centred nursing care to clients with complex geriatric and cognitive needs.
- Coordinate assessment, care planning and ongoing management within a specialist outpatient setting.
- Provide clinical leadership, education and support to the multidisciplinary healthcare team.
- Lead and contribute to quality improvement, clinical audits, research and service innovation.

About You We're looking for an experienced Registered Nurse with advanced geriatric nursing knowledge and the clinical leadership skills to support high-quality care in a specialist outpatient environment. You'll bring strong critical thinking, communication and problem-solving skills, together with a commitment to team-oriented, evidence-based practice.

- Advanced clinical knowledge and experience in geriatric nursing.
- Strong assessment, care planning and clinical decision-making skills.




- Demonstrated ability to work collaboratively within a multidisciplinary team.
- Experience in quality improvement, clinical audits and evidence-based practice.
