Community Registered Nurse

Ario Care Group Pty Ltd is seeking an experienced Community Registered Nurse to provide nursing care and clinical oversight to participants with psychosocial disability and co-occurring complex health needs across Brisbane and surrounding areas.

This is a community-based role for an RN who is comfortable working autonomously, exercising sound clinical judgement and translating complex health needs into practical, safe and person-centred supports within participants' homes.

Our participants may require ongoing support with chronic health conditions alongside their psychosocial disability, with a particular focus on diabetes management, continence and bowel care, wound and pressure care, medication management and other participant-specific clinical needs.

You will work directly with participants while also providing clinical guidance to the wider care team, including support workers, families, allied health professionals, medical practitioners and support coordinators.

About the Role

As a Community Registered Nurse, you will assess participant health needs, develop and review nursing and clinical support plans, provide nursing interventions and support frontline workers to safely implement participant-specific care.

A key part of the role is recognising how physical health conditions interact with a participant's psychosocial needs, daily routines, capacity and independence.

The position also includes a paid on-call clinical component of 14 hours per fortnight, primarily to provide medication authorisation and clinical guidance for the administration of high-alert medications in accordance with participant-specific medication plans and established procedures.

Registered Nurses who would like additional hours will also have the option to pick up support worker shifts with participants, providing greater flexibility and the opportunity to work more directly with the people they support. These shifts are optional and separate from the nursing responsibilities of the position.

You will also identify changes in health status or risk and determine the appropriate response, including reviewing a support plan, providing further worker education, escalating to another health practitioner or initiating an urgent clinical response where required.

Key Responsibilities

- Complete comprehensive nursing and health assessments within participants' homes and community environments.
- Develop, implement and regularly review individualised nursing and clinical support plans that reflect each participant's health needs, preferences, routines, abilities and goals.
- Support participants with psychosocial disability and complex health needs, with a particular focus on diabetes and insulin management, bowel and continence care, wound and pressure injury care, and medication management.
- Provide participant-specific nursing care in other areas where required, including subcutaneous medications, urinary catheter care, enteral feeding, respiratory support, tracheostomy care and dysphagia management, within professional scope and demonstrated competency.
- Establish clear clinical instructions covering routine care, monitoring requirements, expected observations, warning signs, escalation pathways and emergency responses.
- Provide rostered on-call clinical support for medication-related matters, including the authorisation of high-alert medications in accordance with participant-specific medication plans, clinical instructions and organisational procedures.
- Provide timely clinical guidance to support workers where clarification or nursing authorisation is required before medication administration.
- Monitor changes in blood glucose levels, skin integrity, continence, bowel function, medication response, nutrition, hydration, respiratory status and other participant-specific clinical indicators.
- Recognise deterioration, complications, adverse reactions, infection or other emerging clinical risks and take appropriate action.
- Review clinical supports when a participant's health status, treatment, medication, equipment or support requirements change.




- Administer medications and nursing treatments within professional scope, relevant clinical instructions and medication authorities.
- Provide participant-specific education, instruction and practical training to support workers responsible for implementing clinical support plans.
- Assess whether support workers can safely and consistently perform participant-specific clinical tasks before they undertake those supports independently.
- Provide further instruction, supervision or competency reassessment where participant needs change or additional support is required.
- Establish appropriate delegation, supervision and escalation arrangements according to the participant's needs and the worker's demonstrated capability.
- Ensure workers understand their responsibilities, relevant clinical risks and when nursing or other health practitioner input is required.
- Maintain oversight of clinical equipment, medications, consumables and emergency arrangements relevant to participant care.
- Promote appropriate infection prevention and control practices within participants' homes and community settings.
- Work collaboratively with GPs, hospitals, pharmacists, allied health professionals, specialists, families and other members of the participant's support network.
- Maintain accurate clinical assessments, nursing plans, progress notes, medication records, competency records, handovers, incident documentation and clinical review records.
- Contribute to incident reviews, clinical risk assessments and improvements to participant care where nursing or health matters are involved.
- Support participants to understand and participate in decisions about their health care, including raising concerns and requesting changes to their supports.
- Where desired, undertake additional rostered support worker shifts and provide day-to-day participant support in accordance with the participant's support plan and the responsibilities of that shift.

About You You will be a Registered Nurse who combines strong clinical judgement with an understanding that effective community nursing involves more than treating an isolated health condition.

You should be comfortable supporting people whose physical health needs may exist alongside psychosocial disability, cognitive impairment, communication barriers or challenges with maintaining consistent health routines.

We are looking for someone who:

- Is confident assessing and managing chronic and complex health conditions in a community setting.
- Has strong knowledge of diabetes management, including blood glucose monitoring, insulin administration and recognising hypo- and hyperglycaemia.
- Is comfortable providing clinical guidance and medication authorisation remotely when rostered on call.
- Is comfortable assessing bowel and continence needs and identifying changes that may require clinical review or escalation.
- Can assess, manage and monitor wounds, pressure areas and risks to skin integrity.
- Understands medication management, high-alert medications, adverse reactions and appropriate escalation.
- Can translate clinical information into clear and practical instructions for support workers.
- Recognises deterioration and knows when and how to escalate concerns.
- Understands professional scope of practice, delegation and supervision.
- Can teach clinical skills in a practical and understandable way.
- Can assess whether another worker has demonstrated the knowledge and practical skills required to safely provide participant-specific support.
- Communicates effectively with participants, families, support workers and health professionals.
- Maintains accurate and timely clinical documentation.

Essential Requirements
- Bachelor of Nursing or equivalent recognised qualification.
- Current unrestricted AHPRA registration as a Registered Nurse.
- Minimum 1 years' post-registration nursing experience.
- Current First Aid and CPR certification.
- Current NDIS Worker Screening Check.
- Infection control training certificate.
- National Police Check.
- Blue Card/Working with Children Check where required.
- Current Australian driver's licence.
- Reliable vehicle and willingness to travel between participant locations.
- Availability to participate in the agreed on-call medication authorisation roster.

Highly Regarded Experience Experience in the following areas will be particularly valued:
- Community or disability nursing.

Experience with other complex supports such as urinary catheter care, enteral feeding, respiratory or tracheostomy care, dysphagia management, subcutaneous medications or seizure management will also be highly regarded. You do not need to have experience across every clinical area. We are particularly interested in nurses with strong clinical judgement who understand their own scope of practice and can maintain and develop their competencies as participant needs change.
