Position Overview

Ausnew home Care is seeking a suitably qualified and experienced Community Worker to join our team on a full-time basis. The role involves providing structured support services to individuals and families, assisting them to access appropriate community resources and services in line with assessed needs.

Key Duties and Responsibilities

- Assess client needs and provide tailored support to access health care, housing, employment, education, and social services.
- Interview clients to identify social, emotional, financial, and personal challenges and develop appropriate support plans.
- Plan, develop, and deliver community education, training, and support programs to promote wellbeing and independence.
- Work collaboratively with families, community organizations, health professionals, and government agencies to provide integrated, client-centered services.
- Refer clients to relevant service providers and ensure follow-up to support continuity of care.
- Monitor client progress and evaluate the effectiveness of programs and interventions, recommending improvements where required.
- Maintain accurate and confidential client records,



case notes, and reports in line with organizational and regulatory requirements.
- Promote social inclusion, equity, and respect for cultural and individual diversity.
- Advocate for clients’ rights and support their participation within community and service systems.
- Support clients in developing life skills, building independence, and engaging with the community.

Selection Criteria

- A relevant qualification in Community Services, Social Work, or a related discipline, or equivalent relevant experience
- Demonstrated experience in a community services or support role
- Sound communication, interpersonal, and organizationalskills
- Ability to work both independently and as part of a team
- Knowledge of community support systems and service referral pathways
