Greatness starts here Working as part of the Nutrition and Dietetics team, and reporting to the Nutrition and Dietetics Team Leader, you will be responsible for:





Providing client centered care with a high standard of clinical services and expertise to clients with culturally and linguistically diverse backgrounds. As part of this role your day-to-day responsibilities will include;

- Working as an active member of the Nutrition and Dietetics team, applying evidence based, clinical skills and knowledge in the management of a diverse caseload with focus on goal directed assessment and client directed care planning
- Working in a multidisciplinary team to develop and implement person centred care plans, case reviews and initiating referrals as appropriate
- Be involved in quality assurance and service development projects to enhance innovative and efficient practices
- Meeting organisational and funding requirements and KPIs
- Supervision of Dietetic students
- Actively engage in team meetings and case discussions both within the Nutrition and Dietetics team and the Health and Wellbeing team.

Desired Skills and Experience

Bring your great, everyday! To be successful in this role you will bring demonstrated experience working within a multidisciplinary team managing complex clinical presentations in clients with chronic medical conditions and physical and/or psychosocial disabilities.

You will possess a range of skills and requirements, including;

- Qualifications in Nutrition and Dietetics and be eligible for the Accredited Practising Dietitian Program
- A minimum of 3 years working as a Dietitian, preferably in a community setting working with NDIS, HCP, CHSP, HACC and CH funding.
- Experience working with people with chronic health conditions, physical and/or psychosocial disability.
- Experience working in paediatrics
- Well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with your clients.
- You will have proven ability to deliver a service in line with best clinical practice.
- Experience in planning, implementing, and evaluating innovative, evidence based,



clinical service
- Autonomous, with positive caseload and time management skills.
- Enjoy working within a multidisciplinary and interdisciplinary team environment.

The fine print

- Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC and NDIS Clearance.
- Full driver license and travel between sites required.
