My Guardian in Sydney is seeking a Clinical Care Coordinator to lead the clinical heart of our growing home care service. You will conduct home assessments, design individualised care plans, and oversee wound care, medications and allied health coordination.

As an AHPRA-registered nurse, you’ll mentor staff, ensure quality, safety, and NDIA compliance, and build solid relationships with clients, families and hospitals. A supportive team and development opportunities await.
