What You're Will Be Doing

- Provide high-quality nursing care and home visits within the community

- Complete client intake, comprehensive nursing and risk assessments

- Develop, implement and regularly review individualised care plans

- Administer medications, treatments and other nursing interventions

- Coordinate care with clients, families, Support Workers and other healthcare professionals





- Provide clinical guidance, education and competency assessments to staff

- Review incidents and ensure appropriate clinical follow-up

- Maintain accurate and timely clinical documentation

- Conduct quality checks, clinical audits and compliance reviews

- Support compliance across NDIS, Support at Home, DVA and other community programs

- Proactively identify opportunities to improve clinical quality and service delivery

What We're Looking For

- Current AHPRA registration as a Registered Nurse (Division 1)

- Minimum 2 years' experience in community nursing, disability and/or aged care

- Strong experience in clinical assessments and care plan development

- Understanding of clinical compliance, quality control and auditing

- Highly autonomous, organised, proactive and self-driven

- Ability to manage a community caseload with minimal supervision

- Excellent communication and clinical judgement

- Ability and willingness to undertake home visits

- Current driver’s licence
