About the Role

The Accoras Team-oriented Aftercare Program (ACAP) provides non-clinical community-based mental health support to children and young people with severe mental illness and reduced psychosocial functional capacity who are not eligible for the NDIS and other government programs. ACAP is delivered through provision of client-centred outreach interventions which are age appropriate, focused on emotional resilience and skill building, and respond to both the mental illness and related psychosocial impairments.

*The role is a fixed term position up to 30 June 2027*

About You

Your leadership, experience and dedication to supporting young people at risk will be highly valued.

Key responsibilities will include:

- Overseeing and intaking for the ACAP including building community partnerships, and supporting the Service Manager to achieve defined program goals.




- Building and maintain effective relationships with key stakeholders involved in the provision of health, mental health and psychosocial services to young people.
- Actioning all referrals into the program.
- Conducting ongoing risk assessments including assessment of suicide and deliberate self-harm risk and violence risk, developing action plans to mitigate concerns, and providing follow up support and referral for treatment.
- Participating in case review meetings, review progress and exit strategies.
- Contributing to program service delivery planning, evaluation, performance and data reporting to internal and external stakeholders.
- Providing supervision and mentoring to your team.
- Ensuring quality standards are practiced and that opportunities to improve or streamline systems and processes are acted upon.

Qualifications and Experience:

- Tertiary qualifications in Social Work, Human Services, Psychology, or a related allied health discipline; or equivalent work experience.
- Relevant experience working in a case management role delivering psychosocial supports to young people at high risk of suicide.
- Demonstrated skills in making appropriate decisions and problem solving, including in crisis situations and where risk to client safety is involved.
- Highly developed communication,



interpersonal and leadership skills, including the demonstrated ability to network and develop relationships with community partners and stakeholders.
- A high level of attention to detail anddemonstrated strong time management and organisational skills.
- High levels of professionalism, confidentiality and discretion and the ability to relate to and work effectively with young people and their support networks.
- Sound emotional resilience including the ability to sit with discomfort, distress and people in crisis.
