We are looking for a Support Worker (SW) who is compassionate, hard‐working and dedicated to join our team. It is a rewarding job that will give you the opportunity to make a positive contribution to the community, whilst also developing personal skills.

Your key role:

n

As a SW, you will provide support to NDIS-funded participants, promoting choice and control whilst assisting participants achieving their personal goals.

n

Requirement:

n

Our participant requires an experienced Support Worker.

n

A health-related qualification or prior experience supporting participants with complex needs is essential.

n

A successful candidate must have:

n

n
- Experience minimum of 2 years
n
- Excellent verbal and written communication skills.
n
- Confident and a friendly can do attitude.
n
- Ability to communicate sensitively and appropriately.
n
- Understanding of safe work practices.
n
- Previous employment experience in the health/Disability care sector.
n
- Ability to work independently and accurately report back to supervisors.
n
- Flexibility to work various shifts, including evenings and weekends.
n
- Experience in providing quality services that assist people to maintain their independence, dignity, cultural values and religious beliefs.
n




- Experience supporting participants under a Positive Behaviour Support Plan (PBSP) and understanding the importance of minimising and reporting Restrictive Practices in line with organisational and NDIS requirements.
n

n

Duties and Responsibilities:

n

n
- Provide high-quality support to participants which ranges in their level of needs.
n
- Monitoring mental health status.
n
- Undertake company training within the required timeframe.
n
- Keep accurate and detailed documentation.
n
- Work independently within the community or the participant's home.
n
- Reporting after each shift.
n
- Accurate and timely incident reporting in line with organisational requirements.
n
- Support participants to achieve positive outcomes of their personal goals as stated within their NDIS plan.
n
- Transport participants to a range of different services and appointments.
n
- Monitor participant's health and well-being, whilst also contributing to their skill development.
n
- Other tasks as requested by the manager.
n

Qualifications:

n

n
- Support worker: At least 2 years experience (Preferred)
n
- First Aid & CPR Certification
n
- Working with Children Check (Preferred)
n
- NDIS Worker Check (NDISWC)
n
- Driver's Licence
n

n

Experience:

n





Mental Health Support Work: minimum 2 years

n

Requirements:

n

n
- Minimum 3-4 weekdays of availability for day, evening and weekend shifts
n
- Must have or be willing to obtain the following:
n
- NDIS Worker Check (Yellow Care)
n
- NDIS Worker Orientation Module
n
- Working with Children Check (Blue Card)
n
- Valid CPR and First Aid Certificate
n
- Valid Police Check
n
- Current Driver's License, a reliable motor vehicle (including valid registration and fully comprehensive insurance)
n
- Ability to start as soon as possible
n
- Current Australian Driver's License
n
- A personal phone to report case notes and receive shift notifications
n
- Be empathetic, compassionate and patient
n
- Must be able to establish valuable rapport with participants and available to assist with transport to and from appointments and community access.
n
