Registered Nurse – Clinical Coordinator (Home Care) Potts Point, Sydney Part -Time or Casual

An established home care provider with decades of experience is seeking a Registered Nurse to join its clinical team in Potts Point.

This is a hands‑on community‑based role combining clinical care with coordination responsibilities. You’ll support clients living at home, working closely with families, GPs, hospitals and allied health providers to ensure care is calm, consistent and well‑coordinated.

The position offers flexibility and would suit a nurse seeking part‑time work, a short‑term contract (3–6 months), or a role that provides variety without hospital shift pressures.

Requirements

- Conduct in‑home clinical assessments
- Develop and review care plans
- Coordinate services across aged care and hospital interfaces
- Liaise with healthcare providers and families
- Maintain clear clinical documentation and compliance standards

The role requires travel to clients’ homes using your own car for which you'll be paid 0.99 cents per km plus paid travel time. Casual and permanent part‑time opportunities available across South West Sydney & Southern Highlands, where you will provide essential and practical care to help older people live independently in their homes and directly improve their quality of life.

The care work you'll be doing may include:





- Providing personal care – helping with showering and dressing
- Helping around the home – meal preparation and light house cleaning
- Providing transportation to appointments and assisting with shopping
- Assisting with medication

Eligibility

- Registered Nurse with current AHPRA registration
- Minimum 1 year post‑registration experience
- Experience in aged care, community or hospital settings
- Strong communication and organisational skills
- Comfortable working independently in the community
- Driver’s License required
