Join CatholicCare Central Queensland as a Referral Officer and play an important role in helping older people access the services and support they need. You’ll be the first point of contact for new clients, managing referrals, coordinating initial appointments and ensuring each person receives a welcoming and seamless introduction to CatholicCare.

This is a outstanding opportunity for someone who enjoys a combination of customer service, administration and client coordination, and who takes pride in getting the details right.

You’ll work closely with our Aged Care team, clients and other internal teams to make sure referrals are followed up promptly and clients are connected with the right services.

About the role:

As our Referral Officer, you’ll help make the first step into CatholicCare's aged care services simple and positive for our clients.

You’ll manage incoming referrals, communicate with prospective clients, coordinate initial intake appointments and ensure information is accurately recorded and passed to the right team. No two days will necessarily be the same, so you’ll need to be organised,



responsive and comfortable managing multiple priorities.

You'll be an important link between clients, referral partners and our Aged Care teams, helping people navigate the intake process and connect with services that meet their needs.

Key responsibilities

- Manage incoming referrals from referral portals, email and other sources, ensuring they are reviewed and allocated promptly;
- Contact prospective clients to confirm referrals, provide information and coordinate initial intake appointments;
- Create and maintain accurate client records and preliminary profiles within our client management system;
- Record relevant information and case notes while maintaining confidentiality and handling sensitive client information appropriately;
- Follow up prospective clients and help ensure referrals progress through the intake process;
- Identify opportunities to connect clients with other CatholicCare services that may support their needs; and
- Work collaboratively with the Aged Care team and other internal teams to provide a smooth and responsive client experience.

What you’ll bring

We're looking for someone who is organised,



approachable and confident working with people and information.

- Experience in administration, customer service, intake, referrals or a similar client-focused environment;

- Strong communication skills and the ability to communicate professionally with clients, families and team members;
- Excellent organisation and attention to detail, with the ability to manage competing priorities and a high volume of enquiries;
- Experience using client management systems, databases or similar systems, with confidence learning new technology;
- The ability to handle sensitive and confidential information appropriately; and
- A genuine commitment to providing a positive and respectful experience for every client.

Desirable:

- Certificate III in Community Services, Aged Care or a related field, or equivalent relevant experience;
- Experience in aged care, community services, health or a similar workplace;
- Knowledge of aged care funding and referral pathways, including My Aged Care; and
- Experience managing referrals or intake processes.

The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
