Australian Unity is seeking a Care Partner to support customers in their care journey, helping them stay independent and live safely at home or in a preferred setting.

The role is permanent full time with versatile schedule and a hybrid work model (clinic visits, branch days, and WFM). You will coordinate holistic care plans, collaborate with clinical teams, families and external providers to deliver person-centred care and optimise outcomes.
