Job Description
Lutheran Services is seeking a compassionate Care Partner to support older Australians living at home, building trusted relationships and delivering person-centred care. In this part time, permanent role you will help clients live independently with dignity and coordinate care plans within budget.

You will meet clients, develop and monitor plans, liaise with families and providers, manage budgets and maintain accurate documentation in line with aged care standards.
