As our practice continues to grow, we are looking for an
Aboriginal and Torres Strait Islander Health Worker or Practitioner
to join our supportive team and help us continue providing
patient-centred, culturally respectful care for First Nations patients in our community.
This is a part-time position working 3 days per week at Yeppoon Family Practice.

You will work alongside experienced GPs, Nurses and Administration staff, with the support of a practice group that has proudly served the Central Queensland community for 50 years.
This role provides an opportunity to build strong and respectful relationships with First Nations patients and families and support access to
patient-centred, culturally respectful primary healthcare.

About the role
Supporting First Nations patients to access and navigate primary healthcare services
Building strong, respectful and trusting relationships with patients and families
Supporting patients with chronic disease management and preventative healthcare
Assisting with health assessments and ongoing care planning
Supporting the delivery and coordination of
care plans and chronic disease management
Providing patient education, health promotion and lifestyle support
Supporting patients to understand and engage with their healthcare plans
Liaising with GPs, Nurses, allied health providers and external services to support coordinated care
Identifying barriers to healthcare access and helping patients navigate appropriate services
Supporting follow-up, recalls and continuity of care
Assisting with referrals and connection to relevant community and health services
Maintaining accurate and confidential patient records
Contributing to initiatives that support culturally respectful and accessible healthcare for First Nations patients
What we're looking for
Certificate II, Certificate III or higher qualification in Aboriginal and/or Torres Strait Islander Primary Health Care
Genuine commitment to providing patient-centred and culturally respectful healthcare
Strong communication and interpersonal skills
Ability to build respectful and trusting relationships with patients and families
Ability to work collaboratively as part of a multidisciplinary healthcare team
Strong organisational and time management skills
Confidence using computers and clinical software
A positive, flexible and proactive approach
Commitment to maintaining patient confidentiality and skilled standards
Desirable, but not essential




Previous experience working in general practice or primary healthcare
Experience with
care planning and chronic disease management
Experience supporting health assessments and preventative healthcare
Experience working with First Nations patients and communities
Knowledge of local health and community services
Current CPR certification
