We're looking for an experienced
Senior Case Manager
(Empowering Family Connections )
to join our
Intercept Community Services
team, supporting young people and their families through complex, real-life challenges.

Location:
Caboolture, QLD
n Role Type:
Part Time | Mon to Thu | 9am - 5pm | 60 hrs fortnight
Remuneration Range:
$***** - $***** ph + Super + Salary Packaging+ Employee discounts

Intercept Community Services,
Empowering Family Connections program is a targeted family support program that works with young people and their families to strengthen connections and support them to prevent entry or re-entry into the child protection system.
n
What You'll Be Doing:
In this role, you'll deliver therapeutic, strengths-based case management, helping young people stabilise, build resilience, and achieve positive outcomes.
n
n
Delivering
end-to-end case management
for young people and families
n
Conducting
risk and needs assessments
and developing tailored support plans
n
Providing
counselling, mediation, advocacy, and family support
n
Supporting
home stability, life skills development, and transitions
n
Working collaboratively with
Child Safety, Youth Justice, Health, Education and other stakeholders
n
Facilitating
individual and group programs
n
Maintaining accurate
case notes, reporting, and data collection
n
Participating in community networks and service development initiatives
n
n
What You'll Bring:
n
n
Degree in Human Services, Social Work, Psychology or related field
n




Strong experience in case management within youth, family or community services
n
Ability to manage complex cases using therapeutic and evidence-based approaches
n
Confidence working with multiple stakeholders and high-risk situations
n
Excellent communication, documentation, and organisational skills
n
Current
Blue Card, NDIS Worker Screening, and QLD Driver's Licence
n
