Mercy Health in Newcastle, NSW, seeks a full-time Care Partner to deliver care management under the Support at Home Program. You will coordinate with participants, supporters and interdisciplinary teams to enable seniors to live safely and independently at home.
The role focuses on wellness, reablement and culturally secure care, with duties across Restorative Care Pathway and AT-HM. A current NDIS Screening Check is required or in process.
