NSW Health's Hornsby Ku-ring-gai Mental Health Service is seeking a part-time Mental Health Qualified to support Older People with complex needs in home and community settings. You will join a multidisciplinary team delivering evidence-based care for older adults experiencing mental health challenges.
This role focuses on comprehensive assessments, risk management, and coordination with GPs, aged care services, and carers to ensure high-quality support across the care continuum.
