Description: We are currently seeking a Registered Nurse to join our team to provide a range of clinical services to CHADAC clients and to promote health education programs with a key focus to support the GP clinic.
n
Key Selection Criteria
n
n
Current registration with Australian Health Practitioners Regulation Agency (AHPRA)
n
Current First Aid and CPR Certificate
n
Demonstrated knowledge and understanding of Aboriginal and Torres Strait Islander cultures, and the ability to communicate sensitively and effectively with Aboriginal people and communities.
n
Demonstrated clinical knowledge, skills, and experience in delivering evidence-based nursing care within scope of practice.
n
Demonstrated ability to undertake comprehensive clinical assessments, develop care plans, and implement and evaluate care using sound clinical judgement. (Experience with health assessments (MBS 715), GPMPs, TCA's)
n
Demonstrated experience in providing acute and chronic care in a primary health care setting
n
Providing close clinical and administrative support to the GP, working collaboratively to support the delivery of high-quality healthcare to our community.
n
Demonstrated knowledge and experience in the use of clinical information systems (e.g. Communicare) and Microsoft Office applications.
n




Participate in the development of strong and collaborative links with service providers (internal and external) to improve the health status of families and the community on a multidisciplinary basis.
n
At least 2 years post graduate experience
n
Well-developed interpersonal and communication skills, with the ability to communicate effectively and professionally with clients, families, and colleagues.
n

Duties will include (please see position description for full details):

n
Provide culturally appropriate primary health care, health assessment, early intervention, chronic disease management, care coordination, health promotion and health education within a team setting.
n
Conduct holistic health assessments, including Aboriginal and Torres Strait Islander Health Checks (MBS 715), and contribute to GP Management Plans and Team Care Arrangements.
n
Collect and maintain accurate patient information and medical records using Communicare and other designated software programs.
n
Maintain and oversee cold chain management systems in accordance with the National Vaccine Storage Guidelines ("Strive for 5")
n
Coordinate and deliver integrated care in collaboration with GPs, allied health, Aboriginal Health
Workers/Practitioners,
and external service providers.
n
Any other duties as directed by the Health Manager.
