Social Support Officer
Hourly rate: $***** per hour plus 25% casual loading
Hours: wide-ranging
The Town of East Fremantle Neighbourhood Link is instrumental in delivering a social support service to a variety of CHSP clients who depend on the program for help with some routine needs, social engagement and home assisted tasks.
We have a casual position as Social Support Officer to engage and assist our clients on a one-on-one basis and assist with our delivery of a social group activity program.
Job Description
About the position
You will perform tasks related to the delivery of routine personal support to clients in their own home, to CHSP performance Standards.
You will adhere to a support plan that has been developed for the service user by Neighbourhood Link.
Provide a high level of customer service both internally and externally.
Monitor and report to the CHSP Manager any changes in the condition of the service user.




Assist with duties for the physical and emotional care and safetyof clients for (e.g. mealtime assistance and talking/listening.)
Assist in the delivery of social and mentally stimulating activities that promote wellness.
Desired Skills and Experience
About you
Your will have working knowledge of CHSP Standards, or similar.
Excellent organisational and time management skills.
Previous experience with aged or disabled clients, particularly those with dementia.
Excellent communicator that can adapt a communication style for their client.
Be an energetic personality with the strength and stamina to assist with physical supports of clients.
You will hold a First Aid and CPR qualification.
You will hold a current valid C class licence.
