About you:

In your new role you will…

- Deliver quality, patient focused assessment, intervention and case management, utilising specialist contemporary knowledge and clinical experience, for the management of the frail and elderly across the care continuum, with the primary focus of establishing sustainable supports for the prevention of functional decline and avoidance of hospital admission and re-admissions.
- Contribute to an creative evidence-based model of health care for aged care clients with complex health care needs.




- Provide quality Occupational Therapy specific clinical advice regarding patients to professional and operational supervisors, relevant service managers and other stakeholders regarding qualified standards and clinical service development.
