Where you'll be working
Join the Older Persons Mental Health (OPMH) Unit at Westmead Hospital, part of Western Sydney Local Health District (WSLHD). You’ll work alongside a well-supported multidisciplinary team delivering person-centred, recovery-focused care to older people experiencing complex mental health and ageing-related needs.

WSLHD provides primary, secondary and tertiary healthcare services across Western Sydney and the Greater Western Region, with a strong focus on collaboration, respect, empowerment, innovation and quality.

- Work within an acute inpatient OPMH environment supporting complex consumers and their families and carers.

- Play a key role in timely, safe discharge planning and community reintegration, including residential aged care placement.

- Partner with clinicians, community services, government and non-government agencies to build coordinated care pathways.

- Contribute to a culture of learning, professional development, service improvement and evidence-based practice.

- Make a meaningful difference to consumers, families and communities across Western Sydney.

What you'll be doing
As a Social Worker Level 3, you’ll provide advanced clinical social work assessment and intervention for older people experiencing complex and severe mental illness alongside ageing-related and psychosocial challenges. You’ll bring strong clinical reasoning,



advocacy and collaborative practice to every stage of the consumer’s recovery journey.

- Deliver comprehensive biopsychosocial, trauma-informed and recovery-based assessments and interventions for consumers, families and carers.

- Lead and coordinate discharge planning, community reintegration and referral pathways, including complex residential aged care placement.

- Apply advanced clinical judgement and specialist knowledge of older persons mental health to support protected, person-centred care.

- Work across multidisciplinary teams and service networks, including supporting Mental Health Review Tribunal and Guardianship processes and relevant reporting.

- Contribute to supervision, education, quality improvement, research and service development that strengthens outcomes for consumers and carers.

Essential Requirements:

- Qualifications: Tertiary qualifications in Social Work

- Registration: Eligibility for Australian Association of Social Work (AASW) membership

Selection Criteria:

1. Substantial experience and skills in Social Work assessment, planning and evaluating interventions for older consumers with complex mental health conditions and comorbidities.

2. Ability to work independently and as part of a multidisciplinary team

3.



Demonstrated working knowledge of NSW Mental Health Act 2007 and NSW Trustees and Guardian Act 2009.

4. Demonstrated advanced clinical knowledge, skill, and significant clinical experience in in professional and clinical supervision, and support of staff.

5. Demonstrated ability to independently problem solve, apply professional and ethical boundaries when working with complex situations as part of a multidisciplinary team.

6. Demonstrated ability to initiate, lead, complete and evaluate quality improvement initiatives and/or research focused on patient/consumer outcomes.

7. Class C Driver’s License
