Illawarra Shoalhaven Local Health District (ISLHD) is seeking a Registered Nurse - Discharge Planner to support patients with complex discharge needs and coordinate services across the hospital and community care settings.

You will identify discharge requirements early, collaborate with the care team, and advocate for person-centred planning to improve the patient journey from admission through reintegration into the community.
