The Department

Health of Older People provides specialised subacute care for older adults requiring further medical assessment, rehabilitation, and support with complex discharge planning. Our services are designed to promote independence, dignity, and quality of life for patients in later stages of care.

We are currently seeking a committed and experienced Clinical Support and Development Nurse to join our team on Ward ACG—a dynamic unit delivering high-quality, patient-centred care to older adults with complex needs.

Ward AC1

Ward AC1 is a 30-bed unit comprised of a Geriatric Evaluation and Management (GEM) service, focused on subacute rehabilitation for older adults following acute hospitalisation.

Patients on Ward AC1 often present with complex medical, functional, cognitive, and psychosocial needs. The unit provides a supportive and collaborative care environment, where staff work closely within a multidisciplinary team to develop and deliver personalised care plans aimed at maximising recovery and quality of life.

The Role

Every Alfred Care Group nurse must practice in accordance with the Nursing Professional Practice Framework. The Professional Practice Framework defines the elements of nursing practice that improve the care and experience of our patients and aligns with the Patients Come First Strategy. It is comprised of six domains that are crucial to maintain standards, lead change, and add value to healthcare. These domains are Comprehensive Care, Workforce, Leadership, Professional Development, Research and Informatics.

As a clinical nurse, your practice will lead to excellence in patient care. You understand the importance of establishing a relationship between the patient and yourself in order to achieve the best possible outcomes for your patients. You demonstrate the core nursing values of patient centred care, trust and respect in every interaction with patients, carers and colleagues.



You are accountable for establishing and evaluating the patient’s plan of care in partnership with the patient, their carers and other members of the health care team.

Job Description

We are seeking a compassionate and experienced Registered Nurse to join our Homecare team on a casual basis , supporting the delivery of high-quality clinical care to clients living independently in the community.

In this role, you will play a key role in providing clinical support to Homecare Support at Home package clients. This role is ideal for someone who enjoys a mix of clinical work and stakeholder engagement. Your day would look like this:

- Deliver high-quality, person-centred nursing care to clients in their homes
- Conduct assessments and make recommendations for supports, review care plans to meet changing client needs
- Provide clinical guidance and respond to changes in client health status
- Collaborate with multidisciplinary teams to ensure optimal care outcomes
- Build strong relationships with clients, carers, GPs, and community stakeholders
- Ensure compliance with Aged Care Quality Standards, WHS, and organisational policies

About You

You are a dedicated and proactive Registered Nurse who is passionate about supporting older people to live independently and bring the following skills:

- Strong clinical knowledge and sound judgement
- Excellent communication and relationship-building skills




- The ability to work autonomously and within a team
- A genuine commitment to quality care and continuous improvement

Key Requirements

- Bachelor of Nursing with current, unrestricted AHPRA registration
- Minimum 5 years’ post-graduate experience (aged care, community, or hospital setting)
- Experience in wound care, catheter care, assessments, and care planning
- Solid understanding of aged care standards, WHS, and risk management
- Current First Aid Certificate and National Police Check
- Valid driver’s licence and access to a reliable vehicle

About the Role:

- Support and promote activities that are consistent with the Nature and Directions of Beechworth Health Service (BHS)
- In the absence of the Nurse Unit Manager or Assistant Nurse Unit Manager, undertake those duties needed for efficient use of resources that contributes to quality of care of clients.
- Act as a clinical resource, supervisor and role model in clinical practice for Enrolled Nurses, Personal Care Assistants and other care givers.
- Assess, plan, implement and evaluate patient/resident/client care with and in collaboration with other members of the health team. Involve the patient/resident/client or their representative.
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse
- Competent with the Australian Nursing and Midwifery Accreditation Council (ANMAC) Standards.
