Job Description
MannaCare is seeking two Senior Care Partners to join the Support at Home team on a permanent full time basis. One role starts Oct and the other in Mar , reporting to the Home Care Team Lead to supervise Care Partners and support older people living safely at home.

You will coordinate care, develop individualized plans, monitor budgets and participate in Restorative Care, End-of-Life and AT-HM supports, with a focus on quality and compliance.
