Job Description
Monash Health in Melbourne, VIC, invites applications for a Grade 3 Social Worker in the Youth Mental Health Unit 2 at Dandenong Hospital.
The role is full time, Monday to Friday, focusing on young people aged * with acute and complex mental health needs.
You will assess, intervene, advocate and plan discharges within a multidisciplinary team.
You will lead complex discharge planning, work with families, coordinate services (housing, NDIS, AOD, family violence) and supervise junior social
