Australian Vietnamese Women Association is seeking a Care Partner to lead care management under the Support at Home Program.
You will collaborate with participants, supporters and interdisciplinary teams to coordinate tailored, responsive and inclusive support.
You will plan, monitor and coordinate services including AT-HM, focus on wellness and reablement, and ensure culturally secure care across streams.
