Swan Hill District Health is seeking a part time Healthcare worker to join the Support at Home team. The role focuses on community-based services, offering 1:1 supervision in clients' homes and delivering individual care plans to help clients live independently for as long as possible.

Working hours are 0730 hrs to 1530 hrs. You bring a Cert III in Individual Support, a current driver’s license, and a commitment to diversity and client dignity.
