Job Description
Carrington at Home is seeking an organized Case Manager to deliver person-centred in-home care across the Macarthur region from Grasmere.
You will coordinate Aged Care services, support independence, and work within a multidisciplinary team.
You will have qualifications in Aged Care or community services, knowledge of My Aged Care and CHSP/HCP programs, and a patient-centered approach.
This is a full time role with a local office base and client visits.
