Job Description
Australian Unity is seeking a Care Partner in Dubbo to support customers living safely at home or in choosing a supported setting.
You will develop holistic care plans, coordinate multidisciplinary services, and ensure person-centred support across the care journey.
This Permanent Full time role offers adaptable schedule (8:30am–4:30pm or 9:00am–5:00pm, Mon-Fri), hybrid work with 2 days at home and 3 days in the Dubbo office, and a remuneration of $* per hour plus superannuation.
