Western Sydney Local Health District in Granville is seeking a Mental Health Clinician (Registered Nurse) to deliver intensive, recovery-focused care in the community.
You will work with a multidisciplinary team to assess and coordinate care for people with severe and persistent mental illness.
This permanent full time role involves outreach, care planning with consumers and carers, documentation, and collaboration with GPs and NGOs, with Monday to Sunday hours 8:30 am–5:00 pm from October .
