Role: Disability Support Worker and Community Nurse (In- home support)

Disability Support worker Role:

We are seeking passionate support workers with community-based and/or aged care experience. Clients like routine and get comfort out of seeing the same people at the same time, so you will be assigned specific clients, and providing the services at the same time either on a weekly or fortnightly rotation. Reliability is KEY.

We are looking for a support worker for morning care for clients in the Warrnambool region, with shifts starting from 8 am to 11 am ,7 days a week.

You must be available for this morning care for a minimum of 3 -4 days a week.

- Helping with Morning care (personal hygiene, fresh clothing, breakfast preparation)

- Medication reminders

- Toileting assistance

- Fall prevention and monitoring

- Emotional companionship

- Documentation and reporting of any behavioral or health changes

- Meal preparation and food handling

- Driver’s License & Reliable Car is a mandatory requirement

Qualifications and Experience:

To be successful in this role, you will ideally have:

- A Certificate III, IV or higher qualification in Aged Care/Disability

- A current Police Check

- A current First Aid and CPR

- WWC and NDIS Screening Check

- A valid Australian driver’s license





- Medication Administration Training

- Strong interpersonal and communication skills

Due to the personal care needs of the clients, this role requires a female support worker in accordance with the Sex Discrimination Act 1984 (Cth), Section 30.

Community Nursing Role:

Our in-home Nurses deliver specialized, high-quality nursing care to older Australians in client homes in our local communities.

As a Community Nurse, you will provide person-centered care to clients living independently in the community. You will be responsible for:

- Conducting comprehensive clinical assessments and developing individualised care plans.

- Delivering 1:1 in-home nursing care, including wound care, catheter care and insertion, and safe medication administration.

- Supporting Home Care Package clients to maintain independence and enhance their wellbeing.

- Working collaboratively with clients, families, and multidisciplinary teams to deliver evidence-based, high-quality care.

About You:

We are looking for someone who is not only clinically capable but also compassionate, empathetic and committed to making a difference. You will have:

- Current AHPRA registration as a Registered Nurse.

- Experience in aged care or community nursing.

- Robust assessment, planning and clinical skills.

- Excellent communication and interpersonal abilities.

- A current driver’s licence and reliable vehicle.
