About the role

This role is responsible for managing a caseload of older people receiving services under the Support at Home Program, coordinating their care and services, monitoring changing needs, and ensuring each care recipient receives safe, appropriate and person-centred support. The successful applicant will combine their clinical knowledge with strong care coordination and relationship-management skills to support care recipients to remain secure, independent and connected within their own homes.

Key responsibilities

- Manage an allocated caseload of Support at Home care recipients

- Develop, review and maintain individualised care and support plans

- Complete regular client reviews and identify changes in care needs, risks or circumstances

- Coordinate services with care recipients, families, support workers, allied health professionals and external service providers

- Undertake nursing-related assessments and monitoring within scope of practice

- Coordinate referrals for nursing, allied health, Assistive Technology and Home Modifications (AT-HM), and other approved supports where required

- Maintain accurate and timely case notes, assessments, care plans and supporting documentation

- Follow up incidents, clinical concerns,



hospital admissions and changes in a care recipient's health or circumstances

- Communicate effectively with families, representatives, suppliers and other healthcare professionals

- Participate in case conferences and multidisciplinary care planning

- Escalate clinical risks and safeguarding concerns appropriately
