Job Description
Tweddle Child & Family Health Service in Footscray, VIC, is seeking a Family & Community Connections Coordinator four days per week.
You will provide clinical coordination across Tweddle programs, ensuring seamless referral to discharge and robust family support pathways.
You will lead the Assessment & Intake and Home Visiting Teams, give clinical advice for high-risk families, strengthen referrals, and build partnerships with families, communities and external providers to improve outcomes.
