Calvary is seeking a Clinical Project Lead - Dementia Model of Care to drive implementation across hospital, aged care and home care services in NSW.
You will work with consumers, families, clinicians and leaders to ensure consistent, person-centred dementia care and to embed the model of care.
You will lead project activities, stakeholder engagement, education and capability-building, monitor readiness, address barriers and drive sustainable practice change in a large, not-for-profit health
