Job Description
BlueCare is seeking a Clinical Care Partner to coordinate complex clinical and care needs for community clients across New Farm, Hamilton, Kedron, McDowall and surrounding areas.
You will lead assessment, care planning, and service coordination to achieve optimal client outcomes.
The role requires an RN or Allied Health professional with AHPRA registration, a current driver's licence, and the ability to work on the road.
