CQ Medicentre South is seeking an experienced Chronic Condition Management Nurse to join our established and highly regarded practice in Rockhampton.

About the Role n

This position has a robust focus on Chronic Condition Management and preventative healthcare initiatives.

n

Responsibilities include:

n

n
Coordinating Chronic Condition Management plans
n
Conducting Care Plan Reviews
n
Completing Health Assessments
n
Patient education and health coaching
n
Recall and reminder management
n




Working collaboratively with GPs and allied health providers
n
Monitoring patient outcomes and follow‑up activities
n
Maintaining accurate clinical documentation
n

About You n

We are seeking someone who is passionate about patient‑centred care and enjoys helping patients achieve better long‑term health outcomes.

n

You will have:

n

n
Current AHPRA registration as a Registered or Enrolled Nurse
n
Current CPR certification
n
Experience in General Practice nursing
n
Demonstrated experience with or the ability to learn: Chronic Condition Management Plans, Care Plan Reviews, Health Assessments, Patient follow‑up programs.
n
Excellent organisational and communication skills
n
Full Australian working rights
n
Experience using Best Practice software will be highly regarded.
n
