Job Description
Central Highlands Rural Health (CHRH) is seeking a Care Partner to join our Home Support Program across Macedon Ranges and Hepburn Shire.
You will coordinate services, support goal achievement, and provide a person-centred care experience that enables clients to live safely and independently at home.
