You will coordinate care planning, documentation, and reporting for CHSP and related programs, while managing a caseload to meet budget requirements.
You will bring a Certificate IV in Community Services or equivalent, robust MS Office skills, and excellent communication and organisational abilities to deliver high-quality client care.
