Summary:

The Support Coordination Team Leader will play a pivotal role in enhancing the quality of social care services in Ballarat and Bendigo. This position is essential for leading a dedicated team of support coordinators, ensuring that clients receive tailored support to navigate their care options effectively. The Team Leader will foster a collaborative workplace, driving best practices and ensuring compliance with relevant regulations, ultimately contributing to the well-being of the community.

Responsibilities:
Lead and manage a team of support coordinators, providing guidance and support to ensure high-quality service delivery.
Develop and implement individual support plans in collaboration with clients and their families.
Monitor and evaluate the effectiveness of support services, making adjustments as necessary to meet client needs.
Ensure compliance with all relevant legislation and organisational policies.
Facilitate training and career development opportunities for team members.
Build and maintain strong relationships with clients, families,



and external stakeholders.
Conduct regular team meetings to discuss progress, challenges, and strategies for improvement.
Prepare reports and documentation as required for internal and external stakeholders.

Qualifications:
Bachelor’s degree in Social Work, Psychology, or a related field.
A minimum of 5 years’ experience in social care or support coordination, with at least 2 years in a leadership role.
Strong understanding of the National Disability Insurance Scheme (NDIS) and relevant legislation.
Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
Proven ability to lead and motivate a team, fostering a positive and productive work workplace.
Strong organisational skills and the ability to manage multiple priorities effectively.
Proficiency in using case management software and Microsoft Office Suite.
